PREMIUM HEALTH CONSUMER HEALTH DATA PRIVACY POLICY

This Consumer Health Data Privacy Policy supplements the Premium Health Privacy Notice, available here: https://premiumhealth.us/privacy-notice/. This Consumer Health Data Privacy Policy applies to personal data defined as “consumer health data” subject to the Washington My Health My Data Act and the Nevada Health Privacy Act and similar U.S. laws governing consumer health data.

What is Consumer Health Data?

Consumer Health Data” is personal information that is linked or reasonably linkable to a consumer and that identifies the consumer’s past, present, or future physical or mental health status.

Our Collection of Consumer Health Data
To provide the products, services or features offered by Premium Health Management Inc. and its affiliated medical group[s] (“Company”, we”, “us”, “our” and such products, services or features, the “Company Offerings”), we collect and process information about you, including information that could be considered Consumer Health Data. The information we collect depends on how you interact with us and use the Company Offerings, as described further in the “Collection and Use of Information” section of our Privacy Notice.

Depending on how you interact with the Company and the Company Offerings, we may collect the following categories of Consumer Health Data: 

    • Information about your individual health conditions, treatment, diseases or diagnosis
    • Information about your social, psychological, behavioral, and medical interventions
    • Information about health-related surgeries or procedures
    • Information about use or purchase of prescribed medication
    • Bodily functions, vital signs, symptoms, or measurements relating to a health status
    • Information about diagnoses or diagnostic testing, treatment, or medication
    • Gender-affirming care information
    • Reproductive or sexual health information
    • Biometric data
    • Precise location information that could reasonably indicate your attempt to acquire or receive health services or supplies
    • Data that identifies you seeking health care services
    • Other information that is derived or extrapolated from non-health information that we process to associate or identify a consumer with the data described above

Sources of Consumer Health Data

Depending on how you interact with the Company and the Company Offerings, we may collect Consumer Health Data about you directly from you (which may include your interactions with our website and the Company Offerings), from other people (including other users of the website and Company Offerings), your device(s), and partners, vendors and third parties, as described further in our Privacy Notice.

 Why We Collect and Use Consumer Health Data

We collect and use Consumer Health Data as reasonably necessary to provide you with the Company Offerings you have requested. This may include:

    • Providing, personalizing and improving the Company Offerings
    • Providing measurement, analytics and other business services
    • Promoting safety, integrity and security
    • Communicating with you
    • Other essential business operations that support our ability to provide you with the Company Offerings, such as understanding (in aggregate) usage of the Company Offerings and conducting research and development

Disclosure of Consumer Health Data

    • People and accounts with which others share or reshare content about you
    • Apps, websites, and third-party integrations on or using the Company Offerings

How to Exercise your Rights

Depending on where you live and subject to certain exceptions, you may have certain rights in relation to your Consumer Health Data, including the right to:

    • Request that we confirm whether we are collecting or disclosing your Consumer Health Data (and with whom), and to access such Consumer Health Data
    • Request the deletion of your Consumer Health Data
    • Request to withdraw your consent for the collection or disclosure of your Consumer Health Data
    • Review and request changes to your Consumer Health Data
